For many South Africans living with severe obesity, bariatric (metabolic) surgery is a safe, evidence-based treatment that does far more than help with weight loss. It can improve diabetes, high blood pressure, sleep apnoea, fatty-liver disease and overall quality of life. But who truly qualifies—and how do we decide which operation is the best fit?
In this post, I’ll unpack the current criteria and outline how we tailor procedures to the individual, drawing on international guidelines and our team’s long-standing experience in Durban.
The modern eligibility guidelines (it’s not just about a number on the scale)
In 2022, the ASMBS and IFSO updated global guidance to broaden access based on strong outcome data. In short:
-
BMI ≥35 kg/m²: Surgery is recommended, regardless of other conditions.
-
BMI 30–34.9 kg/m²: Surgery can be considered when metabolic diseases (like type 2 diabetes) are present and not well controlled. Dr Gert Du Toit – Specialist Surgeon+1
These criteria recognise obesity as a chronic, relapsing disease where surgery is a proven medical therapy—not a shortcut.
Candidacy isn’t “one-size-fits-all”
Beyond BMI, we look at:
-
Metabolic health: Diabetes control, blood pressure, cholesterol and fatty-liver disease.
-
Risk profile: Surgical risk, medications, previous abdominal surgeries.
-
Lifestyle factors: Nutrition, readiness to change and follow-up support.
-
Goals and preferences: For example, prioritising diabetes remission vs. maximum weight loss.
A structured, multidisciplinary assessment ensures the operation chosen aligns with your health profile and goals. Dr Gert Du Toit – Specialist Surgeon
Choosing the right operation
Different procedures offer different balances of weight-loss power, diabetes impact, nutritional risk and “reversibility.”
-
Sleeve Gastrectomy (SG): Removes ~80% of the stomach to reduce hunger and improve hormonal signalling. It’s effective, with relatively simple anatomy and strong results for many patients. Dr Gert Du Toit – Specialist Surgeon
-
Roux-en-Y Gastric Bypass (RYGB): Both restricts intake and changes gut hormones; excellent for reflux control and diabetes remission in appropriate candidates. Dr Gert Du Toit – Specialist Surgeon
-
Duodenal Switch / SADI-S: Powerful metabolic effect and weight-loss potential for selected patients with very high BMI or difficult metabolic disease—requires diligent lifelong nutritional follow-up. Dr Gert Du Toit – Specialist Surgeon+1
Your surgeon will discuss trade-offs—like reflux history, diabetes goals, medication needs and supplement commitment—before recommending a path.
Why experience and accreditation matter
Technique and team experience influence outcomes. The Durban Metabolic Surgery Centre—led by Dr Gert du Toit and colleagues—has been recognised for advanced metabolic surgery capability, including complex procedures like BPD-DS, reflecting robust systems and expertise. netcare.co.za+1
